This policy explains how NeuroBalance Chiropractic handles information related to this website and inquiries, including Facebook and Instagram lead forms. It does not replace a Notice of Privacy Practices for patient health records. Contact our office with questions about the privacy of clinical information.
Information you provide
When you contact us or submit an inquiry, we receive the contact information you provide, such as your name, email address and phone number, along with your message. Our Meta lead form also asks whether you are ready to discuss scheduling a first visit at our Chapel Hill office.
Please do not submit symptoms, diagnoses, medical history, insurance information or other sensitive medical details through Facebook or Instagram lead forms. Contact our office for instructions on sharing clinical information through the appropriate intake process.
How we use inquiry information
We use inquiry information to respond, discuss our services and help coordinate a first visit. We may also send service-related or administrative communications. An inquiry is not a confirmed appointment and does not establish a treatment relationship.
Facebook and Instagram lead forms
When you submit a lead form on Facebook or Instagram, Meta processes the form through its platform and provides your responses to NeuroBalance Chiropractic. Contact fields may be prefilled from your profile; please review them before submitting.
Our use of the information we receive is described here. Meta handles information on its services under its own Privacy Policy. Submitting an inquiry asks us to follow up about that inquiry; it is not a separate opt-in to an automated promotional text-message program.
Website analytics and cookies
This website uses Google Analytics to understand website activity. It records page visits and interactions such as clicks on appointment links and telephone links. Google Analytics uses cookies and related technologies and receives technical information about visits, such as the page address and browser or device information.
See how Google uses information from sites that use its services for details and privacy controls. You can manage cookies through your browser settings. Our website also loads a SKED review display; that service receives the technical requests needed to show the reviews.
Service providers and sharing
Service providers support functions such as website hosting, analytics, communications, scheduling and intake. Our website links to ChiroHD and SKED for scheduling or intake. When you follow those links, review the privacy information on that service before submitting personal or clinical information.
We may disclose information when required by law, regulation or legal process. Consistent with our communication privacy policy, we do not sell or distribute your contact information to third parties for their marketing purposes.
Protection and your choices
We use reasonable measures to protect information against unauthorized access, disclosure, alteration or destruction. No online transmission or storage method can be guaranteed completely secure.
You may choose not to submit a lead form and contact our office directly instead. To correct your contact information, ask about information you submitted, or request that we stop non-essential follow-up, use the contact details below. You may also follow the opt-out instructions in a communication.
If you separately opt in to text notifications, messages may include appointment reminders, updates and service information. Message frequency varies; message and data rates may apply. Reply STOP to opt out or HELP for assistance, or contact the office.
